What begins in the operating room or ICU should not be abandoned at the discharge door.
Our practice focuses entirely on post-acute settings — skilled nursing facilities, rehabilitation centers, and long-term care communities. By concentrating our expertise here, we've built relationships with staff, developed familiarity with the particular rhythms of recovery, and refined an approach that treats each resident as a person first and a patient second.
We are not a replacement for primary care, nor an extension of the hospital. We are something quieter and more specific: the physicians who are there, on the floor, week in and week out, when continuity matters most.
